I'm about to lose my patience (Witcher 3)

I'm wondering, am I doing something wrong, is this a glitch, do I just suck that much?

How the hell do people recomeend this game on the second to highest difficulty setting?! I am always immensly outleveled, health regeneration items don't do shit, I have quite a lot of alchemy items but can't craft anyhting useful, my equipment is completely damaged already, the leveling is horrible (how the fuck am I suppose to do anything when every quests recomeended level is at minimum twice the amount of mine, same goes for the enemies though not as extreme…), quests give almost zero xp,…

I simply cannot enjoy this game, I already lowered from the second hardest to normal difficulty and I still almost can't complete any quests. Farming may be the solution, but the enemies are usually hard as fuck to beat and give you very little xp.

How the fuck is it acceptable to give out level 16 quest in an area where you arrive at level 3-5?!??

As far as combat goes I take advantage of every sign, I dodge, roll and I'm careful with my timing. Basically similar to Bloodborne where I back of and wait for the enemy to do his move and I slowly slash away at it's health, hitting and backing of.

Am I missing something here?
