Just how random are the locations of (non-witcher gear) sword and armor diagrams?

I just want to make certain of the rules and mechanism. These rules I’m fairly certain of:

a. The locations for most of the sword and armour diagrams are random; some people will get the diagram for sword X from this treasure chest, while others will get the diagram from another chest. b. Some chests have a higher chance of yielding a diagram than others i.e. the chest in Freya’s garden

I’m not very sure about these. I read them on the comment sections, or gleaned from personal observation; I’m hoping someone can confirm whether they are true:

c. If you open a chest that is supposed to have a diagram – according to most people and guides – and do not find one, you can reload and come back; the content of the chest will then change slightly and you will find a sword or armour diagram inside.
d. Some chests will only have diagrams if your level is high enough e.g. if you open a chest at level 14, you will anything but a diagram. However if you open the same chest at level 20 or 30 something, you will then find a diagram. e. Sometimes you can loot a chest and find a diagram which you already have. In other words you can find the same diagram twice. Same rule applies to alchemy recipes.
