The Witcher 3 crashing to desktop?

I've been playing the Witcher 3 since August with no issue up until recently. In the past week it'll take about 5-20 minutes for the game to crash to desktop. It doesn't matter if I am playing or in conversation. Sometimes it'll have the messages "The Witcher 3 has stopped working". Most times there'll be no error message at all.

My rig is an Intel i5 4690k, EVGA NVIDIA GTX 970, 16GB of RAM. I am able to play the game at 60 FPS with high settings and I've had no issue up until now. Now it's literally unplayable because the game will potentially crash as soon as I load up a save.

I have tried rolling back drivers, reinstalling the game, lowering graphics settings and ensuring my GPU is not overheating.

I have found very little about this issue online and so this kind of my last ditch effort to find a solution.

My question is are there any known fixes or does anyone have any suggestions?
