I understand why it's failed, because Spoiler, but "The Battle of Kaer Morhen" had recommended level 24, and Berenger's Blade had recommended level 27, but it turns out they have to be done in the opposite order!
I was quite proud of my Death March run without any failed quests, now I've gotten all the way to finishing "The Battle of Kaer Morhen" and even Spoiler and now I have to do both those parts all over again to keep my quest log clean.
This is my first time playing an RPG and I have to say it's the most frustrating part: having the choice of what order things are done, but knowing that there is actually a hidden order they are supposed to be done in, otherwise they fail (the first was failing "On Death's Bed" because I left White Orchard). I thought the "recommended level" was a hint for that purpose, but it turns out not.
