Game asset files would suggest that this sword exists, but it has not been found to date. The only way to obtain it is to use various mods.

Edit: Actually, it drops in the Arena.

Edit 2: I have the Witcher 2 Enhanced Edition, and after Act II the crafting recipe simply appeared inside my inventory.
