Questions about Adda

I am a bit confused about Adda's fate. In the book The Last Wish, Spoiler.

Then again, in The Witcher 1, we have to lift Adda's curse again. But wasn't it already lifted? I'm a bit confused about de two Adda's and their respective fate. Who died? Why is the curse still here?
